Oriflamme was the personal Imperial-class Star Destroyer of Grand Admiral Octavian Grant. He presumably handed it over to the New Republic when he defected in 6 ABY.
Behind the scenes
Daniel Wallace originally named the Oriflamme in a segment of his Grand Admirals article that was cut. It was eventually officially named such in the first part of the blog series "The Imperial Warlords: Despoilers of an Empire," focusing on the various warlords active after the death of Palpatine. In the real-world, Oriflamme is the symbol of the Abbey of St.-Denis in Paris.
