Could you please give more information on Mon Mothma's worries and the meeting in which she thinks about the Confederacy of Corporate Systems?

I think there's only enough information to say the CoCS was formed by 5 ABY rather than definitively in 5 ABY. She is worried about their rise in 5 ABY, but that doesnt tell us with certainty when the group was formed.
