This incident was an attempt at Jedi Master Yoda's life by Asajj Ventress that took place on the planet Ithor sometime during the Clone Wars, between 22 BBY and 19 BBY. It was thought that the Grand Master had perished in this event for a short time before the truth came out.

This incident was mentioned in passing by Supreme Chancellor Palpatine in 19 BBY in regards to intelligence he felt he was always slow to receive from Jedi Master Mace Windu.
